• Please review your appointment paperwork. You may need to arrive early. Double check to make sure you have your ID and INSURANCE CARDS and other necessary documents.
• Please notify us within 72 hours if you plan to travel long- distance.
• Please notify us if your appointment has been re-scheduled or cancelled as soon as possible.

Please be ready 1 hour before your scheduled pick-up appointment time.

All weather conditions are monitored. If the conditions are not safe for travel, you will be notified on the contact information we have on file to reschedule your appointment.

• Please prepare yourself accordingly for weather conditions, this can include but not limited to: umbrella, poncho, light sweater/ jacket, hat, non skid shoes.

• able to walk independently, or
• able to walk with assistance of a walker or other device, or
• able to stand pivot/ transfer from a wheelchair to a vehicle, and/or transfer from a vehicle to a wheelchair without assistance.
